About one in six Hall County residents is 65 or older: 10,529 people as of July 1, 2025, a 9.2% increase since 2020 (Census Bureau). In the city of Grand Island, about 7,800 people are 65 or older (American Community Survey, 2020 to 2024). Some reach 65 still working, whether at a big plant, a small shop or their own operation. Do you need Part B at 65 if you work in Grand Island?
It depends on who pays first, and that depends on the size of your employer. Medicare's rule is simple. At 65 or older, group coverage through a current job pays first when the employer has 20 or more employees, and second when it has fewer.

One exception helps small businesses: an employer with fewer than 20 workers that is part of a multi-employer plan where at least one employer has 20 or more is treated as the larger size. When does your Medicare sign-up window open and close?
It opens three months ahead of your birthday month and runs seven months in all: those three months, your birthday month and the three after it. Sign up in the three months before your birthday month and coverage starts the first day of your birthday month. Sign up in your birthday month or later and it starts the first day of the next month.

How you sign up depends on what you already receive:
- Already getting Social Security: enrollment happens on its own, and the Medicare card shows up roughly three months before you turn 65.
- Not yet drawing benefits: the online application is at SSA.gov/medicare/sign-up. For an appointment, reach out to Social Security about three months early; SSA.gov/locator finds the nearest office.
- Railroad workers: apply through the Railroad Retirement Board at RRB.gov or 1-877-772-5772.
How should out-of-town specialists shape your first Medicare choice?
Think about them before you choose, because your first choice is the easiest one to make. Plenty of central Nebraskans drive to Lincoln, Omaha or Kearney for a cardiologist, an oncologist or a surgeon. In town, St. Francis is a CHI Health hospital and Grand Island Regional Medical Center belongs to Bryan Health, so even local care can span two systems.
- Original Medicare with a Medigap policy pays any U.S. provider that takes Medicare, so the drive to Omaha or Kearney never raises a network question. Your one-time, six-month Medigap open enrollment window begins with your first month of Part B at 65 or older, and in that window health conditions cannot be used against you. - Medicare Advantage may cost less each month, and Hall County has 21 standard plans for 2027, 13 of them PPOs. Every doctor you see, in town and out of town, needs to be checked by name in the plan's 2027 directory.
There is a safety valve: someone who picks Medicare Advantage at first eligibility and leaves it within a year can return to Original Medicare and buy any Medigap policy with no health questions.
What should Grand Island veterans do at 65?
Look at Part B even if the VA handles most of your care. The two programs split the work. VA benefits pay for care at VA sites, such as the Grand Island VA Medical Center, plus outside care the VA authorizes ahead of time. Medicare pays non-VA doctors and hospitals that take it. VA coverage does not count as employer coverage, so delaying Part B can bring a late penalty if you add it later.

Hall County's Area Agency on Aging is Midland, based in Hastings (402-463-4565 or 800-955-9714), and it is the area's local partner for Nebraska SHIP (formerly SHIIP). Anyone can also reach SHIP at 1-800-234-7119. In Grand Island itself, the Grand Generation Center (304 E 3rd St, 308-385-5308) is open weekdays for information and assistance, and 1-800-MEDICARE (1-800-633-4227) is available 24 hours a day, 7 days a week (TTY 1-877-486-2048).

Frequently asked questions
Usually not right away, if you are covered by the company's group health plan because you are still working. JBS employed more than 3,600 people in Grand Island in 2020, far above the 20-employee line, so its active-employee plan would pay before Medicare. Confirm with your benefits office in writing. When the job or coverage ends, you have eight months to sign up for Part B without a penalty.
During your Initial Enrollment Period, the seven months around your 65th birthday. Individual coverage, including Marketplace plans, is not coverage from current employment, so it does not give you a Special Enrollment Period to sign up for Part B later. Waiting can mean a gap in coverage and a Part B late penalty of 10% for each full year you could have had it.
You may not need to visit one. Most people apply for Part A and Part B online at SSA.gov/medicare/sign-up. To meet with someone, contact Social Security roughly three months before your birthday; SSA.gov/locator shows the closest office. People already receiving Social Security are signed up automatically and get their card in the mail about three months before turning 65.
It depends on your doctors, medications and budget. With a Medigap policy, out-of-town specialists are not a network question, because it pays wherever Original Medicare pays. A Medicare Advantage plan may cost less each month, but you need to confirm each Grand Island, Lincoln or Omaha doctor is in its 2027 network. Decide while your six-month Medigap window is open, because health questions can apply later.
