In short

  • Saunders Medical Center is Wahoo's critical access hospital, with a 24/7 emergency room, skilled care and long-term care.
  • Medicare covers short-term skilled nursing care after a qualifying hospital stay. It does not cover long-term custodial care.
  • Time under observation does not count toward the three-day inpatient stay Medicare requires before skilled nursing care.
  • Wahoo members of a Medica Medicare Advantage plan need a new plan for 2027, because CMS plan data shows no Medica plans in Saunders County.

Wahoo is the Saunders County seat, about 29 miles and 40 minutes north of Bill Jurey's office in Lincoln on US-77. You can meet him by phone, by video or at the office at 285 S 68th St Place. There is no cost to you, because the insurance carriers pay the agent.

Wahoo had 5,023 residents on July 1, 2025, up from 4,802 in 2020 (Census Bureau). About 18.2% of them are 65 or older (American Community Survey, 2020 to 2024). Saunders County as a whole is 20.7% age 65 and older, and its 65+ population grew 17.8% between 2020 and 2025.

How does Bill work with people in Wahoo?

Mostly by phone or video, with the Lincoln office open when you would rather sit down in person. Bill starts with your medications, then your doctors and hospital, then what a hospital stay or a stretch of rehab would cost you, then your budget. He shows you the options and the math. The decision stays with you.

What should Wahoo residents check about Saunders Medical Center?

Its network status, plan by plan. Saunders Medical Center, at 1760 County Road J (402-443-4191), is a critical access hospital, the federal designation for small rural hospitals. It runs a 24/7 emergency room, skilled care, long-term care and an MS clinic. Its own website does not state a health system affiliation, so you cannot assume it follows any big system's network.

For each Medicare Advantage plan you are considering for 2027, look up Saunders Medical Center by name, then your doctor there, then any Lincoln or Omaha specialist you see. HMOs generally cover only in-network care outside of emergencies, and PPOs cover out-of-network care at a higher cost. If you stay with Original Medicare and add Medigap, any provider that accepts Medicare is open to you. Plan lineups, premiums, provider networks and benefits vary from county to county and from one year to the next.

What is the difference between skilled care and long-term care?

Medicare pays for one and not the other, and a hospital like Saunders Medical Center offers both. That is why this question comes up so often in a town like Wahoo.

The qualifying stay is an inpatient hospital stay of at least three days in a row, not counting the day you leave. Time under observation, or in the emergency room before you are admitted, does not count. If you are in the hospital, ask whether you are an inpatient or an outpatient under observation. Some Medicare Advantage plans waive the three-day rule, so check your plan's 2027 Evidence of Coverage.

The Medicare and long-term care guide goes further, and long-term care insurance explains how people plan for the part Medicare will not pay.

What changes for Wahoo Medicare plans in 2027?

The biggest change is a departure. In the 2027 CMS plan data, Medica no longer offers Medicare Advantage in Saunders County. Wahoo members should read the notice Medica mailed, then choose a replacement between October 15 and December 7, 2026. If returning to Original Medicare appeals to you, the exit lets you apply for certain Medigap policies without health questions, provided you apply inside the federal time limits.

If you take costly specialty medications, look closely at drug coverage. The Part D out-of-pocket cap on covered drugs is $2,100 in 2026 and rises to $2,400 in 2027. The Part D out-of-pocket cap guide explains what counts toward it.

If hitting that cap in January or February would strain your budget, ask your drug plan about the Medicare Prescription Payment Plan. Every Part D plan and every Medicare Advantage plan with drug coverage offers it. You pay $0 at the pharmacy and get a monthly bill instead, spread across the rest of the year. It does not lower your total cost, and if you change plans you have to sign up again with the new one. The payment plan guide covers who it helps.

Frequently asked questions

Check before you enroll, because it varies by plan and year. Search the plan's 2027 provider directory for Saunders Medical Center, then call the hospital to confirm. Check its clinic doctors and its skilled care unit too, plus any Lincoln or Omaha specialist you see. Do not rely on a system name, since the hospital's own website does not state a system affiliation.

Medicare pays only for short-term skilled care, not long-term custodial care. After an inpatient hospital stay of at least three days in a row, Part A can cover skilled nursing facility care, up to 100 days per benefit period, when you need daily skilled care or therapy. Help with daily activities over months or years is usually paid privately, by long-term care insurance, or by Medicaid.

No. Time in the hospital under observation, or in the emergency room before you are admitted, does not count toward the three-day qualifying inpatient stay, and the day you leave does not count either. Ask the hospital staff whether you are an inpatient or an outpatient under observation. Some Medicare Advantage plans waive the three-day requirement, so check your plan.

It does if you are a Medica Medicare Advantage member. Medica's plans drop out of Saunders County in the 2027 CMS plan data. If you choose nothing, you would be moved to Original Medicare without drug coverage. Use Annual Enrollment, October 15 to December 7, 2026, to pick another plan. Returning to Original Medicare instead also gives you a time-limited right to buy certain Medigap policies without health questions; the dates are in Medica's letter.
