Frequently asked questions

It is 7 months long: the 3 months before the month you turn 65, your birthday month, and the 3 months after. If your birthday falls on the 1st of the month, Medicare treats you as turning 65 the month before, so the whole window shifts one month earlier.

Since January 1, 2023, if you sign up in your birthday month or any of the 3 months after it, Part A and Part B start on the first day of the month after you sign up. Signing up in the 3 months before your birthday month starts coverage on the first day of your birthday month.

Not always. If you have health coverage from your own or your spouse’s current job at an employer with 20 or more employees, you can usually delay Part B without a penalty. When that coverage ends, you get an 8-month Special Enrollment Period. Smaller employers often require Medicare to pay first, so check before you wait.

You can sign up during the General Enrollment Period, January 1 to March 31, with coverage starting the month after you sign up. You may owe a Part B late enrollment penalty of 10% for each full 12 months you went without Part B when you could have had it.
